Some of the buildings may be old, but there are modern people living in them and those people need communication services. Hence we see television antennas and satellite dishes clustered across the rooftops.
Whether these are still in use to any great extent is unknown. Obviously there are plans to roll out high-speed broadband across Italy, and as that happens I imagine that free to air and satellite services will begin to fade away as they have done in many other countries.
I tried to find some references on how far high-speed Internet broadband has progressed in Italy, but there is nothing specific about how abundant the services are in Napoli.
