To John G Reed, the commentator below me; they are not spikes, but spurs. They are not sharp (pointed), rather blunt and they are on little 'rollers'. The horse can feel the contact, but it doesn't hurt or damage the animal. Just Google it if you want a closer up image of what it is and how it works.
