Softbox with grid to side with reflector at reasonably far distance behind for small amount of hair reflection.
I was going to see if I could get the old fashion effect. Don't quite have it yet.

I have finally decided that what makes this image work is the reflection in her glasses. It's a clear focal point. Perhaps more detail in her hair would have been good... and overall the image is a bit low key. Maybe just brighten it up a bit, bring the lights in a little closer... the angle is good. She has lovely skin for sidelighting! regards B
