If you're using an Adjustment Layer, this is what the layer structure should look like.

Very simple and if you aren't squeamish about changing the pixels of your image directly,
as I said earlier you could do the same thing without an adjustment layer by using a
direct selection instead. But, I highly recommend using an adjustment layer because
this makes the curve dynamic, allowing it to be changed later if desired.
