Part of being both a good physician and a photographer is the learning of the art of observation, that is, looking intently and keenly at your precise focal point. This photo demonstrates it all, from getting one's eye on the ball, to focusing on the hole, and then, going beyond into the world of x-ray vision. (how else would the good doctor know that there were no "hemorrhoids there.") Of course, it is also necessary, beyond these complexities, to be able to tell a good story. Nicely done.
By the way, when I was about 10 years old, I saw an advertisement in a comic book for "x-ray" glasses, perfect for seeing through women's dresses. After I had sent away for the glasses and received them, I was at first rather disappointed that the glasses had no x-ray powers whatsoever. However, after straining my eyes quite a bit, hoping that somehow the glasses would eventually work, I found myself looking at my subjects in a different light. That was the true beginning to my photography career.
