Lots of PP obviously. Two images combined... one with her facing one way, the other facing opposite. My mother-in-law was kind enough to let me mess up her normally PERFECT guest room. Blood was added in post, lighting adjusted and dodged & burned, her expression was emphasized and hair fixed (with the smudge tool) and a bit of noise added.
