With the shutter set at 5 seconds, I released the shutter after I heard the
firework shot off but before it had actually opened. Thus, any camera shake
from pushing the shutter button was not apparent. Camera was on manual
focus with the lens set to infinity, thus no time lag for focus.

Very nice. I've had a lot of trouble taking fireworks shots with a digital camera. Part of the problem is the noise and part is the limitations on the aperture settings.
I did get some nice shots from the top of the Contemporary Hotel at Disney World. You can see one of the shots at:
The fireworks were from two separate shots and placed behind the Cinderella Castle in Photoshop. You might try placing your shot behind a night shot of some kind. If you place it all the way at the top and have a foreground object block the bottom, the out-of-frame parts won't matter.
