Clean up the background with some cloning of the worst damage, and then Neat Image applied several times to blur and clean up "patterned" dirt.

Lots of cloning on the little boy to get rid of dirt, and one run of Neat Image to clean things up afterwards.

Combined the layers, then used levels to brighten everything just a touch.
