This image was taken with fill flash, using the Canon 580ex with Stofen Omnibounce on it. There we barely perceptible glints in the eyes, which I painted over on a separate layer, after lightening the iris and darkening the ring around the iris with the dodge and burn tools. If you click on "next" you can see how the eyes looked in the original, unedited image. I used no noise reduction on this, although some could probably have been used. The dark shadow under her chin seems to be reflecting color from her shirt - that should probably be dealt with, too, but my purpose in posting this was to show the eyes after editing, in comparison with how they looked in the original image.
