This is probably from the Perkins Sams Pocket, second oxidation zone, thought to be NW stope, Number 2 sub-level, on Level 35, found late 1980. Larger crystals than this one tended to be extensively altered to malachite. I interpret the crystal forms to be {001}, {320} and {101}. Many other azurite crystals from Tsumeb are elongate parallel to the b axis.
