progressive bone loss post loading of implant. Attempted to remove crown with GC pliers, not succesful. Crown removed by drilling through crown and access screw. 5mm+ of one loss around implant, decided to remove implant and bone graft. Implant removed with counter clockwise rotation utilizing implant driver. Site grafted with 50/50 MTF allograft (cortical/cancellous) followed by collagen membrane.
