Atraumatic exo 26, 25 implant placed in the usual manner, care taken to clean out all soft tissue in 26 socket. loss of buccal plate, palatal socket exposure into sinus cavity but no membrane perforation, small perforation of palatal plate (1mm diameter) at apex, 26 osteotomy prepared in ideal position, entry into sinus chamber, no evidence of sinus membrane perforation, packed allograft, good pushback of bone, implant placed, entire socket/defect grafted with allograft followed by collagen membrane (with hold cut for healing abutment) placed over palatal, crest and buccal.
