Select a "mask" outline and create new layer via copy
Free transform mask to enlarge it very slightly.
Adjustment desaturate and lighten
Apply Neat Image to smooth out skin detail
Apply Plastic wrap artistic filter with very smooth settings
Layer style: drop shadow
New Layer for the tie string, drop shadow
New Layer, Soft light blending mode, Painted in some colors on the mask:
Pink cheeks, blue eye shadow, a touch of flesh color on nose and other white mask areas.
