I wanted to be able to mount a Bogen heavy duty flex arm to my Manfrotto dual flash macro bracket. The flex arm lets me position my flash in a wide variety of places.

Here is what the rig looks like after it's all put together.

In actual use, I would have 2 flashes on the bracket, and would use my 90 mm macro lens. Alternately, I might use this rig with my Sigma macro flash.

I did a very crude edit on the picture to remove distracting background elements.
