Canon EOS 5D ,Canon EF 70-200mm f/2.8L IS USM
1/100s f/2.8 at 110.0mm iso800 full exif

At the distance this was shot the total depth of field is around 3 inches so things look pretty soft very quickly.

The focus was so soft I was sure you were using a filter.

I didn't use any filter. Also no light modifiers or reflectors. This is just window light and shooting the 70-200f/2.8L IS wide open.
